NOTICE Effective January 1, 2012, National Vision Administrators (NVA) will no longer be our vision network provider.
Co-op Optical has merged with Davis Vision. All members will utilize the Davis Vision Network Nationwide. Co-op is part of the Davis Vision provider network.
There are no other changes in benefit coverage.
Former NVA members should contact Davis Vision member services at 1-800-282-8975 if you have questions.
For current members already enrolled with Davis Vision the member services line remains unchanged at: 1-800-999-5431.
